If you are a covered employee or dependent who is experiencing a problem getting a prescription filled or refilled or your pharmacy has problems processing a claim, contact CVS/caremark. You can also get more information on CVS/caremark on the Internet.
If you are experiencing difficulties using the Plan's network of health care providers, or wish more information about your home network, contact CARE COORDINATORS.
